Coaching Certification Requirements(Opens in a new window) - A Pupil Activity Permit (PAP) is required for all Reading coaches, whether paid or volunteer. You can access the document that lists all required certifications and trainings to obtain or renew your PAP, along with step-by-step directions for the process.

Fundraising Form(Opens in a new window) - This form must be filled out, submitted to the Jr/Sr High building secretary, and approved by the Jr/Sr High Principal and Superintendent prior to the fundraising activity.

Overnight Trip Form(Opens in a new window) - This form must be completed for all overnight athletic trips and approved by the building principal, superintendent, and the Board of Education. Ensure it is filled out in a timely manner to allow for BOE approval.

Reimbursement Form(Opens in a new window) - Original itemized receipts must be turned in for reimbursement.

Requisition Form(Opens in a new window) - All purchases for our teams (regardless of amount) require an approved purchase order be in place prior to the purchase. The first step to obtain a purchase order is to complete the requisition form and submit it, along with a quote, to the Jr/Sr High building secretary. Only Varsity head coaches may submit a requisition form for their program, regardless of the level the purchase will be for (Varsity, JV, Junior High).
